How the math works
A missed appointment is not a delayed appointment. The hour is gone, the staff were paid, and the patient who would have taken the slot was told you were full. That is why practices count the full visit value rather than a fraction of it.
The recovery line uses the most conservative credible figure in the research. Across 29 studies, automated reminders cut non-attendance by a weighted average of about 29 percent. Live phone calls do better, around 39 percent, and multi channel reminders are reported between 30 and 60 percent. We use the low end on purpose.
Weekly appointments times your no-show rate times average revenue per visit, annualized. The recovery line is 29 percent of that total.
What the research shows
| Measure | What the research shows |
|---|---|
| National average no-show rate | About 18 percent of scheduled appointments, higher in some specialties |
| Automated text reminders | Cut non-attendance by about 29 percent on average across 29 studies |
| Live phone reminders | About 39 percent, at a much higher staffing cost |
| Multi channel reminders | Reported between 30 and 60 percent |
| Cost of one missed appointment | Commonly put at around $200, considerably more for specialists |
Reminder effectiveness figures come from a review published in The American Journal of Medicine and later replications.
Reminders are the floor, not the ceiling
Reminders are the cheapest thing that works, which is why every practice should start there. They are also not the whole answer, because a patient who cancels the night before still leaves you with an empty hour.
What closes that gap is what happens after the cancellation. A waitlist that texts the next patient automatically, self scheduling so people can move their own appointment instead of skipping it, and a deposit on the visit types that get skipped most. Those are scheduling decisions more than software ones, and the software just has to make them easy.
If your practice management system already does all of this well, keep it. We build when the system in place cannot do what the practice needs, not because building is always better.
Questions people ask
What is a normal patient no-show rate?
The national average sits around 18 percent, though it varies widely. Primary care and behavioral health typically run higher, established specialty practices lower. Pull your own rate from the last three months rather than using an average, because the number is the whole point.
How much does one missed appointment cost?
The common figure is around $200, based on average collected revenue per visit. For specialists and procedure based practices it is considerably more. Use what your practice actually collects per completed visit for an honest answer.
Do appointment reminders really work?
Yes, and the evidence is unusually consistent. A review across 29 studies found automated reminders cut non-attendance by about 29 percent, with live phone calls closer to 39 percent. The gains come from timing and from making it easy to reschedule rather than simply not show.
Should we charge a no-show fee?
Some practices do, and it changes behavior. It also creates collection work, awkward front desk conversations, and problems with certain payers, so it is worth checking your contracts first. Most practices see a bigger return from reminders and a waitlist before they get to fees.
What does a scheduling and reminder system cost to build?
It depends on reminders, self scheduling, waitlist logic, and what it has to connect to. You get one fixed itemized quote after a free consultation. If an off the shelf tool covers what you need for less, we will tell you that at the consultation.
